For example: If you’re first visit with Dr. Lewis is an Acupuncture Visit, then click the Acupuncture Patient Intake Button below.
- All New Patients should click and download the Welcome Letter.
- Next, choose the one Patient Intake that corresponds to your visit:
- For any appointment other than Acupuncture, choose the Naturopathic Medicine Intake Button below. Simply click the Buttons.
- The Welcome Letter is for you to print read. It provides insight as to what is expected in your first few visits with Dr. Lewis.
- Each Patient Intake is approximately 6 pages requesting detailed information about your health history. The Intake form is necessary for Dr. Lewis to begin your thorough evaluation. It is required on or before your first visit with Dr Lewis.
- It’s best to print it, fill it out, and scan/email (info@drlisalewis.com) or fax (973-486-0838) it to Dr. Lewis prior to your appointment. This will speed up your treatment and save time if Dr. Lewis can begin studying your case prior to your appointment.
- If you are unable to scan or fax the Patient Profile, please bring the forms with you to your first visit.
